We have been helping existing and new customers with the new Amazon requirements so that they are in compliance and we can surely do the same for you. To expedite your testing requests, we have prepared a few basic submission request forms that we would ask you to complete in advance of sending us your samples for testing. Once we receive and process your submission request, we will be ready to receive your samples for testing.

What do I need to include in my submission to begin testing?

Please provide copies of the below items:

  • Dyad Labs Customer Information Request Form (if new client) (located in the download button above).
  • Finished Product Specification
  • Batch Records (Blending Discharge, Blending Discharge Table and Blending Yields)
  • Input weights of each ingredient + run total weight must be clearly visible.
  • Picture of product (Lot # and Supplement Table)
  • Deviation documentation (if applicable)
  • Manufacture + Expiration date (if applicable)
  • Submission form, “Dyad Amazon Project Submission Form” filled out to completion (located in the download button above).
  • Please enter information to appear EXACTLY as it should on COA.
  • Amazon Project Questionnaire, if needing testing to qualify for new Amazon requirements (located in the download button above).

Rush services (Requires Dyad Labs approval)
